Required Enrollment Documents
In compliance with Florida State Statutes for private schools, Acceleration Academics requires the following documents to be on file for every student prior to their first day of class.
Please upload clear PDF or image copies of the following:
1. Copy of Birth Certificate
Why: To verify legal identity and age eligibility for grade placement.
Requirement: A clear copy of the student's state-issued birth certificate.
2. Immunization Record (Form DH 680)
Why: Florida Statute 1002.42(6) requires all private schools to maintain current immunization records for every student.
Requirement: Form DH 680 (Florida Certification of Immunization), obtained from your pediatrician or local health department.
Exemptions: If you have a religious objection to immunizations, you must submit Form DH 681 (Religious Exemption), which can be obtained at your County Health Department.
3. School Entry Health Exam (Form DH 3040)
Why: Florida Statute 1002.42(5) mandates a school-entry health exam dated within one year of enrollment.
Requirement: Form DH 3040, completed by a licensed healthcare provider within the last 12 months.
4. Custody or Guardianship Documentation
Why: To ensure we are communicating with the legally authorized educational decision-maker.
Requirement: Only required if applicable. If the student is not living with both biological parents, or if there are specific court orders regarding educational rights (e.g., divorce decrees, foster care placement, legal guardianship), please submit the relevant court documents.

We understand that providing immunization and physical exam records for an online program may seem unnecessary since your child is learning from home.
However, Acceleration Academics is a registered private school in the State of Florida. This means we are legally required to maintain the same compliance files as a brick-and-mortar campus. These documents are subject to audit by the Florida Department of Health and the Department of Education to ensure our continued operation. We appreciate your cooperation in helping us remain compliant!
